Trilogy Care and Humility Support Services (HSS) have partnered to ensure you have access to high quality home care services via your Support at Home plan.

Humility Support Services (HSS) is a nurse-led, family-operated provider delivering safe, high-quality in-home care across South-East Queensland. Led by Registered Nurse Khule and Enrolled Nurse Lelo, the team combines strong clinical skills with compassion and cultural respect to help people stay well, independent and connected at home. HSS provides personal care, medication support, wound care, diabetes care, PEG and NGT support, respiratory support, lymphedema care, chronic disease monitoring, meal preparation, household assistance, in-home respite, transport and community access. With ongoing RN oversight, clients receive clinically safe care that can flex as their needs change, including complex-care support

Humility Support Services partners with Trilogy Care to deliver aged care under the new Support at Home program. Trilogy Care helps turn more of each client’s approved funding into real support, with more care hours from the same budget, clear choice over who comes into the home, and support to fully use each quarterly budget. Together, HSS and Trilogy Care focus on humility, strong communication and family collaboration, delivering reliable, culturally safe care across Brisbane, Logan, Ipswich, Redlands and surrounding areas

Trilogy Care empowers you to live independently in the home you love for longer by helping you build and self-manage your care needs. Trilogy Care is an approved Support at Home provider and is registered with My Aged Care to deliver in-home care support
